I know it's many years later, but I combined a few photos that can be found online, and produced a new version. None of the photos available online are in the correct proportions, nor are they cropped correctly to match the movie. In order to make it work, I had to fake some extra background above his head, which you can only see if you look for it. Cloning background from other parts of the photo ended up looking even more fake, so this is the closest I can get it. I think it's darn close. ;-)

Metric size provided, as Hellraiser II was filmed in the UK. I got it as close as I could to something you can put in a standard frame. Judging from movie screencaps, this looks pretty much dead on, although it was necessary for me to zoom it ever so slightly. The casual viewer won't see the difference.
Print it on photo paper at 300 dpi, and fold in half.
